Forms and Information for Teachers in Our 2018-2019 National Pilot Study

Braille Files:

Your student has been provided the graphs and maps in braille. If you feel your student will benefit from hard copies of the text and problems in the units, please contact Dr. Rosenblum to arrange for these.

Teacher Session Recording Forms:

For each of the 10 instructional units, you need to complete a TEACHER SESSION RECORDING FORM. You have 3 options for submitting the forms:

1. Complete the forms in your notebook and email, fax, or US Mail them to Penny Rosenblum* 
2. Download the fillable Word form for the unit your student is completing. Save it to your desktop. Once the form is complete, email it to Penny Rosenblum
3. Download the fillable PDF for the unit your student is completing. Save it to your desktop. Once the form is complete, email it to Penny Rosenblum
